New Student Admission Procedure

Registration forms are to be turned in to the school office. The following items should be furnished at the registration time:
  • Application
  • Application Fee ($150)
  • Testing Fee ($25)
  • Signed Parental Financial Contract
  • Original of current immunization form
  • Copy of social security card
  • Copy of birth certificate
  • Copy of health insurance card *
  • Copy of custody papers, if applicable
  • Parental Commitment Form
  • Family Information Form
  • Report cards for the past two years and current year’s to date grades
  • Achievement test scores for the past two years

* The insurance card is a part of the student file, along with an Insurance Refusal Form, which the parent completes. This form indicates whether or not the parent wishes to take school insurance, which is available at the beginning of the school year.


All of these required documents, along with the $150.00 application fee and $25.00 entrance test fee, must be received prior to scheduling the entrance exam.

Students entrance exams will be scheduled in the order in which documentation and fees are submitted to the School Office. The entrance exam will be set up once the student’s file is complete. The balance of the enrollment fee ($300.00) is due upon notification of the student’s acceptance.

Registration Fee Information (all students):
  • Application fee (registration) will be $150.00 due when you turn in your application.
  • Enrollment fee will be $300.00 and due by March 15, 2008.


Important Registration Dates:
  • Registration forms will be sent home on Wednesday, January 2, 2008.
  • Registration for currently enrolled students will be on Thurs. - Mon., Jan. 3-14, 2008.
  • Open House will be on Thursday, January 10, 2008 from 12:00 to 2:30 p.m.
  • Registration for siblings for currently enrolled students and Cottage Hill Baptist Church members will be on Tues., January 15, 2008.
  • Registration for the public will open on Wed., January 16, 2008 and will close when all classes are filled.
