To request a transcript be sent, please complete a Transcript Request Form. The forms are located on top of the file cabinet outside the Guidance Office door. After completing the request form, place it in the request box located inside the Registrar's Office. Transcript requests should be in the request box by Wednesday for a Friday mailing.
